Hello,
i never used custom-portraits on the PW, although I have some pretty good ones, because I suspected that other players cannot see them anyway.
So i always used default-portraits so far.
My question is simple -->
Can custom-portraits be seen by other players all the time, or not ?

Only those who have the same portrait may see it
Those who do not will see the brownish-black generic portrait in its place

Many people are using the portrait pack from the Steam Workshop.

Yes as others have said - other people can see it, but only if they also have access to the same portrait.
This can mean portraits included in the Thay nwsync, or if multiple people have the same one from another source (as Bertram indicates).
So strictly to your question: "All the time" - likely not, but if you pick the right one, it could be at least most of the time :)
