Don't neglect your saving throws.
Nearly all of my respawn situations have been caused by failed saves. This becomes more of an issue the more advanced you become (higher levels), as you will face an ever increasing number of spellcasting foes (which makes sense, given that this is Thay). Often, one botched save is enough to get you killed. Obviously, you're going to roll a terrible number once in a while anyway, but you can greatly reduce the likelihood of sudden death by having good saves in the first place.
Even trading off a little AC for equipment that boosts your saves might prove worthwhile.
